Neftaly Device Image Upload Sheet (July Edition)
Purpose:
To standardize submission of device photos during July donations—before and after tagging—for accountability, audit readiness, and inventory tracking.
1. Submission Metadata
- Device Asset Tag #:
- Device Type: (e.g., Laptop, Desktop, Tablet)
- Serial Number / Model:
- Staff Name & Department:
- Date of Upload: [dd/mm/yyyy]
2. Photo Upload Requirements
A. Pre-Tagging Images (at least 2)
- Front View: Full image of device before tagging.
- Side/Rear View: Close-up image showing ports, serial number, or wear.
B. Post-Tagging Image (at least 1)
- Tagged Device: Clear image showing Neftaly asset tag attached.
Note: Files should be named AssetTag_front.jpg, AssetTag_side.jpg, AssetTag_tagged.jpg.
3. Upload Instructions
- Accept JPEG/PNG formats only.
- Max file size: 5 MB per image.
- Ensure clear focus and lighting.
- Use form’s upload fields to attach each image separately.
- Mandatory fields: front, side, and tagged images—form won’t submit until all are attached.
4. Additional Information (Optional)
- Comments/Observations:
- Example: “Minor screen scratch on right corner”
- Example: “Port covers missing—needs replacement”
5. Submission Verification (System or Manual)
- Upon upload, system generates receipt confirmation.
- Entry is linked to the corresponding Device Donation Intake Form and Inspection Sheet.
- Missing or unclear images flagged and sent back for re-submission.

- Build the form using platforms like Typeform, Jotform, or Paperform, as they offer image-upload fields, previews, and integration capabilities
- Connect uploads to cloud storage (e.g., Google Drive) for easy access and organization .
- Embed conditional logic to require tagged-image upload only after the asset tag field is completed .
